Homecare
Local and national surveys indicate that, given a choice, most people would choose to die in their own homes. Hospice of Wichita Falls cares for most individuals in their own homes, or in contracted nursing facilities or assisted living facilities. Our homecare program serves all or parts of the following Texas counties: Archer, Baylor, Clay, Foard, Hardeman, Jack, Knox, Montague, Wichita, Wilbarger, and Young.
Each patient is assigned to a team of experts that will be responsible for providing care and support to the patient and their family during their end-of-life journey. Your team of professionals will include a specially trained RN, LVN, Hospice Aide, Social Worker, Volunteer, Chaplain, and Physician. Your team is trained to address the medical, social, emotional, and spiritual needs of the patient and the family.
The hospice team is available 24 hours a day, 7 days a week for support, consultation, and visits. In the nursing facility or assisted living facility, the team becomes an adjunct to the staff, advising, teaching, observing, supporting the patient and family, and providing extra equipment if needed. The Hospice of Wichita Falls RN Case Manager assigned to your team will work with you and the hospice team to identify your desires and needs and ensure that your plan of care will satisfy those needs and desires.
